I recall reading about a court case in California where an executive of a health insurance carrier was on the witness stand. He was asked if his company routinely denies claims without even reviewing the facts, knowing many if not most are legit. He acknowledged this was their standard practice.

Talk about lacking ethics, integrity, or even common human decency…
